FALLS CHURCH, VA — With Fairfax County Health Department's restaurant inspection reports back online, we continue to look at reports in the Falls Church area.

The health department's environmental health staff inspect over 5,200 restaurants and other retail food service establishments in Fairfax County and cities of Falls Church and Fairfax to ensure safe food handling practices are followed. This includes proper sanitation, food storage and preparation, and adequate kitchen facilities. Aside from restaurants, staff inspect child care centers, hospitals, school cafeterias, and temporary food service vendors at festivals.

Until this summer, food establishment inspection reports had been unavailable online since 2019 as the health department worked to launch a new online system. Residents can search for a specific restaurant or browse a map of inspected locations through the online database.

The health department cautions residents that a single inspection should not be used to evaluate a restaurant. Pieter Sheehan, the Fairfax County environmental health director, says "it is important to look at multiple inspection reports for a facility to truly understand their approach and commitment to food safety and sanitation."

This week's report focuses on restaurants in the City of Falls Church through mid-August. Click on each link to see the full report for the restaurant.

Thai Time: 6613 Wilson Blvd, Falls Church, Va 22044

Violations: Food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hourspast the point in time when the food is removed from temperature control: garlic in oil, cooked chicken and panaug curry, dispensing and/or in-use utensils improperly stored between use as follows: in containers of room temperature water, food stored on the floor and/or food stored less than 6 inches off the floor.

New Moon Restaurant: 6619 Wilson Blvd, Falls Church, Va 22044

Violations: Food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: fried rice observed at 99-132F in a large bowl sitting out at room temperature, one or more of the elements of an effective employee health policy is either missing or incomplete, food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: various pupusa fillings at 60-72F observed inside Berg 2DR prep cooler (previously sitting out at room temperature), raw bean sprouts at 56F observed sitting out at room temperature, methods used for cooling were not adequate to facilitate proper cooling: observed large deep bowl of cooked noodles cooling covered tightly in the Walk in Cooler, no temperature measuring device located in the following cold holding equipment: True 2DR Display Cooler, Berg 2DR prep cooler (Chinese side), Berg 2DR prep cooler (Spanish side), dispensing and/or in-use utensils improperly stored between use as follows: sitting in containers of room temperature water.

Violations: Observed the lack of an approved backflow prevention device on the water supply system, person in charge is not a certified food protection manager, food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events, refrigerated, ready-to-eat, time/temperature control for safety food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: boiled eggs, taro in 2dr RIC, dry storage foods were observed stored on ground in boxes, nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: paper boards are used to line shelving and refrigerator, manufacturer containers were observed reused for the storage of other cooked foods.

Saigon Bakery & Deli: 6773 Wilson Blvd, Falls Church, Va 22044

Violations: Handwashing facility located in the kitchen is blocked with a step stool and bulk ingredient containers preventing access by employees for easy handwashing, equipment/utensils were observed soiled to sight and touch: several utensils on the table across from the cookline, refrigerated, ready-to-eat, time/temperature control for safety food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: a tray of sticky rice wrapped in banana leaves cooked 2 days ago in the walk in cooler, food-contact equipment surface of the baking equipment are observed soiled with accumulations of grime and debris.

VietFoods: 6783 Wilson Blvd, Falls Church, Va 22044

Violations: Food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced beef, cooked shrimp in 3 dr prep cooler, left, at cookline: 47/47 degrees F. Bean sprouts in container on counter: 68 degrees F. Bean sprouts, shredded lettuce, cut greens, rice in 1 dr prep cooler near dish machine: 56/53/47/49 degrees F, hand soap was not provided at each handwashing sink, grocery bag was observed reused for the storage of cut greens, refrigeration unit is not operating properly and is unable to maintain cold food at or below 41F: 1 dr prep cooler close to dish machine, dispensing and/or in-use utensils improperly stored between use as follows: small bowl used to scoop rice.

Huong Viet Restaurant: 6785 Wilson Blvd, Falls Church, Va 22044

Lo Dau Hu Thanh Son: 6793 Wilson Blvd, Unit# A, Falls Church, Va 22044

Violations: Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: partially cooked chicken above uncooked fish in walk-in refrigerator, ood employee was observed cleaning their hands in a food preparation sink, warewashing sink or in a mop sink, current person in charge is not a certified food protection manager (certified food manager arrived during inspection).

Banh Cuon Saigon Restaurant: 6795 Wilson Blvd, Falls Church, Va 22044

Thanh Van Restaurant: 6795 Wilson Blvd, Falls Church, Va 22044

Violations: food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked vegetable item 45F, bean sprouts stored in refrigerator 46F, bean sprouts stored with ice 78F, fried tofus stored on ice 60F and 65F, food item(s) for which time rather than temperature is used as a control is not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control: all foods sitting out at room temperature, foods sitting out on counters for customers, following food items that are not easily identified by appearance were observed without a label: squeeze bottles of oils and sauces, containers of seasonings next to stove burners, utensils for rice and soups stored in containers of room temperature water, unwrapped knives, forks, or spoons were not stored with the handles up, food was observed stored directly on the floor in the dining areas and kitchen areas, wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter, EHS observed cleaned and sanitized utensils displayed or dispensed with the food or lip-contact surface facing upward, refrigeration unit is not operating properly and is unable to maintain cold food at or below 41F, observed take home containers displayed with the food or lip-contact surface facing upward.

Mango Mango Dessert: 6795 Wilson Blvd, Unit# 1de, Falls Church, Va 22044

Violations: A certified food protection manager was not present at the start of the inspection (certified food manager arrived during inspection), mop is improperly stored between use - stored in mop bucket after use, floors in the kitchen (near waffle area, by water heater & 2DR upright cooler #1) and walls (near waffle area) are in need of cleaning, exterior of multiple waffle makers, spill tray underneath flipping waffle maker, top and bottom of mixing stand are cleaned as required, newspaper is used under waffle makers, in container that holds food coloring bottles.

Happy Lamb Hot Pot: 6799 Wilson Blvd, Ste# 10, Falls Church, Va 22044

Violations: Observed raw chicken and raw beef stored above squash, 2) Observed raw shell eggs stored above mushrooms, 3) Observed raw calamari stored above corn, food-contact surfaces ran through the chemical dish machine were being sanitized before the cleaning/soap step as the tubing/hoses were connected out of order, observed an unapproved “splitter” attached to the map sink faucet, food item(s) located in the following units were observed cold holding at improper temperatures above 41F using a calibrated food temperature measuring device: Turbo Air 2DR prep cooler, Everest 3DR Display Upright Cooler, Open Display Cooler (customer self service) and new buffet unit, hand soap was not provided at each handwashing sink, EHS observed the Owner installed a buffet, sauce station, self service customer refrigerator and soda fountain drink machine without Health Department review and approval, no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions, pesticides labeled for residential use were observed in the facility, food thawing using an improper method (sitting out at room temperature): fish,shrimp, calamari, lamb loin and noodles, dispensing and/or in-use utensils improperly stored between use as follows: in container of room temperature water, observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees, open drinking containers used or stored in a manner that may contaminate food, food contact surfaces or utensils.

Lesly Restaurant Bar & Grill: 306 Hillwood Ave, Falls Church, Va 22046

Violations: The handwashing sink at kitchen is equipped with separate cold and hot water faucets rather than a mixing valve or combination faucet, temperature of the in-use wash water solution was observed at less than 110F. Hot water was at 97F, refrigerated, ready-to-eat, time/temperature control for safety food that is prepared on site is not used or discarded within 24 hours of preparationand was not observed to be date marked: beans, rice, tamale filling, picketed cabbage in walk-in-cooler, ice machine and cooler doors at bar are broken, light fixture outside of wall-in-area is broken leaving the area dark, light bulb in storage area is out and light inmen’s room is flickering, plumbing connections under the men’s room sink piping are leaking, no covered refuse container for the disposal of feminine napkins in the ladies room stall, openings to the exterior of the building are present along the back door, smoking area is not structurally separate from the non-smoking area.

Preservation Biscuit Company: 102 E Fairfax St, Falls Church, Va 22046

Violations: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/orundercooked: eggs.

Caribbean Plate: 133 E Annandale Rd, Falls Church, Va 22046

Violations: Person in charge is not a certified food protection manager, nonfood-contact surfaces of the following equipment had accumulations of grime and debris: clean cooler and freezer surfaces and other no food contact surfaces regularly, wall around the mop sink is not maintained in good repair, pooled water in 3DR Victory, repair door handle on Maxx Cold freezer, repair prep sink., drain at the 3 compartment sink is slow to drain and there was wastewater observed on top of the grease trap.

Pupuseria La Familiar: 308 S Washington St, Falls Church, Va 22046

Violations: Food-contact surfaces of the following equipment were not observed sanitized: plastic containers, refrigerated, ready-to-eat, time/temperature control for safety food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked: tamales, corn & carrot, other ready-to-eat items in the walk-in-cooler.

Takumi Sushi Bar: 310 S Washington St, Falls Church, Va 22046

Violations: Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: observed various raw animal products stored over cabbage in 2DR True Cooler and raw shell eggs stored over cabbage in 2DR Central Prep (R).

Emily's Restaurant II: 404 S Washington St, Falls Church, Va 22046

Violations: Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: bowl of raw shrimp stored on top of tortilla in 2DR Avantco Prep, hand soap was not provided at each handwashing sink.

Elevation Burger: 442 S Washington St, Falls Church, Va 22046

Violations: Foor gaskets of the following unit(s) are (missing, damaged): bottom drawer on two drawer cooler is not aligned when closed and can leave an opening for warm air to enter.
